Use this section to submit an applcation for COVID-19 Leave Arrangements

Getting started

The updated leave and other arrangements include:

  • employees may access annual/recreation leave entitlements at half pay (application form)

  • employees may access Supplementary Provisions (CD3.1: Employment Conditions – Hours of Work, Overtime and Leave) to receive a lump sum payment in lieu of their accrued annual/recreation leave  (application form)

  • chief executives are able to direct employees to take annual/recreation leave when their individual balance exceeds 12 months’ accrual, with 3 business days’ notice

 

    How to

    Submit forms

    1. Employee to complete and sign the appropriate COVID-19 application/payment form
    2. Send completed, signed COVID-19 application/payment form to Agency delegate

    3. Agency delegate to authorise leave/payment

    4. Submit form via the 'Get in touch' drop-down by selecting your agency

    5. Click on the email and attach the completed, authorised form
